- Online treatment sessions will not be recorded.
- Provider has discretion in determining telehealth appropriateness.
- You agree with provider's assessment and consent to treatment.
- You agree to provide accurate and complete medical information (including from other healthcare providers and emergency contact).
- You understand that inaccurate information can negatively impact treatment.
- Provider may determine that in-person treatment is required.
Consent and Withdrawal
- Consent can be withdrawn at any time with written notice.
- Withdrawal does not affect prior actions or other consent provisions.
- Ongoing use of services implies agreement to telehealth treatment.
Electronic Communication and Privacy
- Services involve electronic communication of personal medical information.
- Information will be handled according to applicable privacy policies.
Financial Considerations
- You may obtain medical examinations from other healthcare providers.
- Prescriptions will be fulfilled by Professional Entities' pharmacy partners and shipped.
- You are responsible for all costs, including prescriptions, and will not seek Medicare reimbursement.
