1. About This Policy

Elewell LLC (“Elewell,” “we,” “us,” or “our”) operates elewell.shop and app.elewell.shop. We offer supplements and connect individuals with licensed healthcare providers and partner pharmacies for telehealth and prescription services.

This policy explains how we collect, use, disclose, and retain personal information and the choices available to you. Our services are offered in the United States to adults age 18 or older.

Our HIPAA Notice of Privacy Practices describes the practices of the entities identified in that notice. Independent healthcare providers and partner pharmacies may also have their own privacy notices. This policy does not replace an applicable healthcare privacy notice or any separately required consent or authorization.

2. Notice at Collection

The information we collect depends on how you interact with us. Health information and certain identification information may be sensitive personal information under applicable law.

Category Examples and purposes Retention criteria
Contact and account information Name, email, telephone number, address, and account identifiers used to maintain accounts, fulfill requests, provide support, prevent fraud, and send permitted communications. Based on the duration of your account or service relationship, communication preferences, and outstanding requests or obligations.
Purchase and payment-related information Purchases, subscriptions, payment transactions, order history, and delivery information used for payments, fulfillment, refunds, support, and accounting. Based on fulfillment, subscription, refund, dispute, tax, accounting, and product-safety requirements.
Online identifiers and website activity IP address, browser and device information, cookie identifiers, website interactions, and advertising or analytics events used for website operation, security, analytics, and advertising. Based on the information’s purpose, applicable system settings, and the period reasonably needed for website operation, security, analytics, and permitted advertising.
Demographic and identity-verification information Date of birth, regional information, government-issued identification, and verification results used to confirm identity, age, and eligibility and prevent fraud. Based on identity and eligibility verification needs, fraud prevention, and applicable recordkeeping requirements.
Health and prescription information Intake responses, medical and medication history, prescription information, and service status used for evaluation, care coordination, fulfillment, patient support, and permitted communications. Based on service and care-coordination needs and applicable healthcare recordkeeping requirements. Clinical records maintained by providers and pharmacies follow their retention requirements.
Communications and preferences Support messages, account and service-status information, and communication preferences used to respond to requests, provide notifications, and administer choices. Based on resolving inquiries, providing related follow-up, honoring preferences, and documenting required consents or authorizations.
Rewards, referrals, and reviews Membership information, eligible purchases, points, redemptions, referrals, and reviews used to administer programs and benefits. Based on program participation, outstanding benefits or disputes, and how long a submitted review remains published.

We disclose online identifiers and website activity to advertising partners for advertising measurement and relevant advertising. These disclosures may constitute a “sale,” “sharing” for cross-context behavioral advertising, or targeted advertising under applicable state laws. You may exercise applicable opt-out rights through Your Privacy Choices.

3. Sources and Uses of Information

We collect information directly from you when you create an account, purchase products or services, complete an intake or verification process, contact us, submit a review, or join a program. We also collect information automatically through our websites and receive relevant information from healthcare providers, pharmacies, payment processors, and other providers supporting our services.

We use information to provide requested services, administer accounts and payments, coordinate fulfillment and care, communicate with you, operate programs, improve our websites, prevent fraud, resolve disputes, and comply with legal obligations.

Marketing and advertising activities are subject to applicable privacy choices and any required consent or authorization.

4. How We Disclose Information

We disclose relevant information to the following categories of recipients:

  • Healthcare providers, clinical networks, and pharmacies for evaluation, treatment, prescribing, dispensing, care coordination, and related professional obligations.
  • Technology, payment, and operational providers for website services, server-side event processing, account administration, payments, subscriptions, fulfillment, shipping, and security.
  • Patient communications and support providers receive relevant contact, order, service-status, health, and prescription information to support patient services, including portal support, shipping and delivery notifications, and refill reminders.
  • Supplement marketing providers receive contact information, supplement shopping and purchase activity, and communication preferences to send permitted supplement promotions. We do not use patient medical or prescription information to select recipients or personalize these promotions.
  • Rewards, referral, and review providers to administer participation and benefits.
  • Analytics and advertising partners, including Google and Meta, for analytics, advertising measurement, and relevant advertising, subject to applicable restrictions and choices.
  • Legal and professional recipients where appropriate for legal compliance, professional advice, dispute resolution, or protection of rights and security.

Recipients’ responsibilities depend on their role, the information involved, and applicable law.

5. Health Information and Communications

Medical information is maintained within our telehealth platform. We share relevant information with service providers supporting care coordination, fulfillment, patient support, and automated communications such as shipping updates, delivery notifications, and refill reminders. Patient support is provided through our patient portal.

Supplement promotional emails are managed separately. We do not use patient medical or prescription information to select recipients or personalize those emails.

Where we disclose protected health information to advertising or analytics partners in reliance on your optional HIPAA Authorization, those disclosures are limited to the information, recipients, and purposes it describes and remain subject to applicable law and your privacy choices. Advertising purposes may include measurement, retargeting, and personalized advertising, as described in the authorization.

The authorization and consent to receive promotional emails are separate and optional. Neither is required to receive services. Providing an authorization does not override your other applicable privacy choices.

You may unsubscribe from promotional emails using the instructions in each message or contact support@elewell.shop. Necessary account, order, and care-related communications may continue where permitted by law.

You may revoke your HIPAA Authorization by following its instructions or contacting care@elewell.shop. Revocation applies prospectively and does not affect actions already taken in reliance on a valid authorization.

6. Cookies, Advertising, and Privacy Choices

We use cookies, pixels, and similar technologies for website operation, analytics, advertising measurement, and relevant advertising.

We also use server-side tracking services, including Stape, to process certain website events on our behalf before forwarding information to analytics and advertising partners. This processing includes filtering or removing certain data fields to limit the information disclosed. Information that remains may still constitute personal information and is subject to the disclosures, restrictions, and privacy choices described in this policy.

You can exercise applicable sale/sharing and targeted-advertising opt-outs through Your Privacy Choices. Applicable advertising opt-outs apply to both browser-based and server-side disclosures. Any disclosure of protected health information remains subject to the requirements described in Section 5.

We honor recognized opt-out preference signals, including Global Privacy Control (GPC), as required by applicable law. A signal may apply to the browser or device sending it and, where required, an account we can recognize. You may need to enable the signal or submit your choice on other browsers or devices.

An advertising opt-out does not necessarily disable essential website functions or every analytics function. It is separate from deleting an account, unsubscribing from promotional communications, or revoking a HIPAA authorization.

7. Relationship with Shopify

The Services are hosted by Shopify, which collects and processes personal information about your access to and use of the Services in order to provide and improve the Services for you. Information you submit to the Services will be transmitted to and shared with Shopify as well as third parties that may be located in countries other than where you reside, in order to provide and improve the Services for you. In addition, to help protect, grow, and improve our business, we use certain Shopify enhanced features that incorporate data and information obtained from your interactions with our Store, along with other merchants and with Shopify. To provide these enhanced features, Shopify may make use of personal information collected about your interactions with our store, along with other merchants, and with Shopify. In these circumstances, Shopify is responsible for the processing of your personal information, including for responding to your requests to exercise your rights over use of your personal information for these purposes. To learn more about how Shopify uses your personal information and any rights you may have, you can visit the Shopify Consumer Privacy Policy. Depending on where you live, you may exercise certain rights with respect to your personal information here Shopify Privacy Portal Link.

8. Rewards, Referrals, and Financial Incentives

Our supplement rewards and referral programs are voluntary and free to join. Where the California Consumer Privacy Act applies, this section serves as our Notice of Financial Incentive.

Benefits and participation. Create or log in to your Elewell account and join through My Elewell Rewards. Members earn 0.5 points per dollar on eligible purchases, 20 points for joining, up to 50 points for eligible reviews, and 5 points each for eligible Facebook and Instagram activities.

Points redeemed Discount Minimum order
50 $5 off $50
100 $10 off $75
150 $15 off $100

Through referrals, a new customer receives 10% off their first order. After the qualifying purchase, the referring member receives a coupon for 10% off a future order. Referral rewards are discount coupons, not cash payments. Eligibility, redemption conditions, and current offers are described on our Elewell Rewards page and in the relevant offer before participation or redemption.

Information involved. We use contact and account identifiers, eligible purchase history, points and redemption records, referral activity, submitted reviews, and records of qualifying social activities to administer the programs, attribute referrals, and provide benefits.

Leaving the program. You may leave at any time by contacting support@elewell.shop. Before your membership ends, you may redeem eligible unused points, subject to the program’s redemption requirements. Unsubscribing from promotional emails does not end your membership or remove your points.

Value of information. Program information helps us maintain customer relationships, attribute referrals, and administer rewards. For the approximately six-month period ending September 22, 2026, our initial good-faith estimate of its value is approximately $22.61 per enrolled member. We use program expenses as a proxy, calculated from approximately $900 in rewards and referral software costs plus $95 in redeemed coupons, divided by 44 enrolled members. Separate review-platform fees are excluded. Benefits vary with participation; we relate them to the value of program information using the costs of providing those benefits and administering the program. This is an estimate for the stated period, not a cash value or payment owed to a member. We may update the estimate as participation and costs change.

9. Retention

We retain personal information only for as long as reasonably necessary for the purposes described in this policy, using the category-specific criteria in our Notice at Collection. Relevant factors include the nature and sensitivity of the information, our relationship with you, and applicable legal requirements.

Particular records may be retained longer when necessary to meet legal obligations, resolve disputes, or address fraud or security incidents. Consent, authorization, privacy-request, and preference records are retained as needed to honor choices and satisfy applicable legal requirements.

When retention is no longer necessary, we delete or de-identify the information in accordance with applicable law.

10. Your Privacy Rights

We offer core privacy-request options to U.S. residents nationwide. Subject to applicable requirements and exceptions, you may request:

  • Information about whether and how we process your personal information.
  • Access to and a copy of your personal information.
  • Correction of inaccurate information.
  • Deletion of personal information.
  • An opt-out of applicable sale, sharing, or targeted advertising.
  • An end to promotional communications.
  • Review or appeal of a privacy-request decision.

Requests may be subject to appropriate verification, legal exceptions, recordkeeping obligations, and the rights of others. We do not unlawfully discriminate against you for exercising privacy rights.

Depending on applicable law, additional rights may concern sensitive information, certain profiling or automated decisions, recipient information, authorized agents, appeals, or regulator complaints.

California Privacy Rights

Where the California Consumer Privacy Act, as amended by the California Privacy Rights Act, applies, California residents have the right to:

  • Know and access the categories and specific pieces of personal information collected about them, the categories of sources, the purposes for collection, and the categories of recipients.
  • Request deletion of personal information, subject to applicable exceptions.
  • Correct inaccurate personal information.
  • Opt out of sale or sharing of personal information.
  • Limit certain uses or disclosures of sensitive personal information, where applicable.
  • Receive equal treatment for exercising their rights.

The Notice at Collection and disclosure sections above describe the information categories, purposes, and recipient categories relevant to our services.

To opt out of applicable sale or sharing, use Your Privacy Choices or a recognized opt-out preference signal such as GPC. To exercise other rights, contact support@elewell.shop.

An authorized agent may submit a request on your behalf, subject to applicable proof-of-authority requirements. Sale/sharing opt-outs are not subject to the same identity-verification process as access or deletion requests.

11. Submitting a Request or Appeal

Email support@elewell.shop and describe your request. For an appeal or review, identify the earlier request and explain why you would like the decision reviewed.

Please do not include medical records, ID images, passwords, or full payment-card details in an initial email. Where verification is needed, we request information reasonably necessary to process the request securely.

We respond within applicable legal timeframes and explain any permitted extension, limitation, or denial. Where an appeal or regulator-complaint process applies, we explain that process.

Requests concerning medical records may be governed by the applicable healthcare notice and the responsible provider or pharmacy. We help route those requests where necessary. HIPAA-related requests may also be directed to care@elewell.shop.

12. Security and Age Restrictions

We use administrative, technical, and organizational safeguards appropriate to the information we handle. No method of transmission or storage is completely secure.

Our services are intended for adults age 18 or older. If you believe a child has provided personal information through our services, contact support@elewell.shop.

13. Changes to This Policy

We may update this policy as our practices or legal requirements change. We update the date above and provide additional notice or obtain consent when required by law.
